Insurance If you have an insurance plan that has orthodontic benefits we shall be happy to assist you in claiming your benefits. To facilitate processing your claim, prior to your initial visit, we will contact your insurance company to determine your available orthodontic benefits. You will not need to obtain an insurance claim form, we have forms we use when submitting. If your insurance must have continual claim forms, we will also do this for you. Payments can be directed to you or to our office. Even though insurance is filed, it is not a guarantee of payment.
